How do I Register for the CCFHS Patient Portal?

You must be an established patient of CCFHS to be eligible to Register for the Patient Portal.

You can obtain Instructions for Registering to the CCFHS Patient Portal by asking us at your next visit.

We will provide you with the instructions and information you will need to register and validate your account on the CCFHS Patient Portal.

Why do I need to go to the clinic to register for the Patient Portal?

When enrolling for the Patient Portal, you must visit the clinic so we may verify your ID. You may be asked to provide a Photo ID for verification. If you wish to register for the Patient Portal, please inform your provider, nurse or the front office personnel that you would like to enroll and we will instruct you on how to do so.

I have forgotten my password to my account.

If you have forgotten your password to your account, you may click 'Forgot Password', on the Home page, and answer the HINT QUESTIONS that you designated to help you reset your password.

You should always safeguard your password to your CCFHS Patient Portal account and never give out the password to your account.

CCFHS physicians and nurses do NOT have access to your password and they will NEVER ask for your password.

Who can see the information I submit on my CCFHS Patient Portal?

Only authorized medical staff that is trained to receive information submitted via the CCFHS Portal has access to your information.  This may include physicians, nurses and scheduling staff.

Why do I need an email address to register?

Each person wishing to register on the CCFHS Patient Portal will need a unique, valid email.  We will use this email to send you a PIN number that you will use for Identity Verification when creating a new Patient Portal Account. The email will also include instructions on how to register for the CCFHS Patient Portal.  Each CCFHS Patient Portal user must have their own unique email address as we do NOT allow emails to be used more than once during registration.(Example: husband and wife CANNOT use the same email to register.)

How do I get an email address if I don't have one?

You typically get a FREE email address from your ISP(Internet Service Provider). FREE email addresses may also be obtained from service providers such as  GMAIL, Yahoo, etc.

Can I view my upcoming appointments?

Yes, you may now view your upcoming appointments, as well as past appointments, once you are registered on the Patient Portal.  You can also cancel or request to reschedule your appointments.

Why can't I see my Immunizations?

Currently, patient Immunizations are not included for viewing on the patient portal. We hope to enable this feature in the near future. If you need your immunization record please call the office or stop by for a copy.
